Care in the Comfort of Your Home
Southland Home Health provides quality, full-service care to patients in the comfort of their homes.
We provide comprehensive, multidisciplinary home health care services to promote, maintain, and restore health. Our goal is to minimize the effects of illness and disability. Our team is comprised of professionals who are dedicated to providing high-quality care every step of the way.
At Southland, our home health care team includes RNs, LPNs, CNAs, and social workers. We offer a rehabilitation team for physical, occupational, and speech therapy. Together, our team collaborates to deliver care tailored to meet the specific needs of our patients.
Our Home Health Disciplines Include:

Behavioral Health Nursing
Our behavioral health nurses approach the care of a patient with mental illness from a holistic perspective, within his/her chosen environment. We aim to make behavioral health care the most cost-effective treatment available for this patient population.
Medical Social Workers
Our medical social workers help with social and emotional concerns of an illness. This includes counseling or offering resources within the community.
Home Health Aides
Home Health aides provide assistance with daily living activities and personal care.
Our physical therapists work to restore the mobility and strength of our patients. Our rehabilitation efforts align with patients recovering from a physical injury or a disability.

Our speech therapists help patients with speech disorders restore their ability to communicate, eat, and swallow.
