Southland Home Health provides quality, full-service health care to patients in the comfort of their homes.
We provide comprehensive, multidisciplinary in-home health care services to promote, maintain, and restore health. Our goal is to minimize the effects of illness and disability. Our team is comprised of professionals who are dedicated to providing high-quality care every step of the way.
At Southland, our home health care team includes RNs, LPNs, CNAs, and home health aides. Together, our team collaborates to deliver care tailored to meet the specific needs of our patients.
Our Home Health Disciplines Include:
Skilled Nursing
We provide a full range of nursing services including wound care, IV therapy, diabetic care, catheter care, cardiac and respiratory assessment, medication management, pain management, and cancer care.
Psychiatric Nursing
Our psychiatric nurses approach the care of a patient with mental illness from a holistic perspective, within his/her chosen environment. We aim to make psychiatric home health care the most cost-effective treatment available for this patient population.
Medical Social Workers
Our medical social workers help with social and emotional concerns of an illness. This includes counseling or offering resources within the community.
Home Health Aides
Home Health Aides provide assistance with daily living activities and personal care.
Physical Therapy
Our Physical Therapists work to restore the mobility and strength of our patients. Our rehabilitation efforts align with patients recovering from a physical injury or a disability.
Occupational Therapy
Our Occupational Therapists help restore a patient’s ability to return to their usual activities of daily living. This may include a self-care routine, meal preparation, and maintaining their home independently.
Speech Therapy
Our Speech Therapists help patients with speech disorders restore their ability to communicate, eat, and swallow.

Disease Management
We provide support and education to patients with chronic illnesses including CHF, COPD, CAD (Heart Disease), Stroke, Hypertension, Diabetes, Ostomy, Enteral and Respiratory with the ultimate goal of avoiding unnecessary rehospitalizations and ER visits.
IV Therapy
Our IV Therapy offerings include administering and monitoring intravenous infusions of fluids and medications.
Wound Care
Our Wound Care program helps prevent wound complications, while promoting wound healing.


Medication Management
As part of our Medication Management program, we work with patients to coordinate, monitor, and assess medications needed for their care.
Diabetes Management
Our Diabetes Management Program helps educate patients and families on the management of blood glucose levels.
Orthopedic Programs
We develop individualized treatment plans to aid in recovery after an orthopedic injury or surgery, such as joint replacement surgery.
Cardiovascular Programs
Our highly-trained professionals collaborate with patients to help them recover from cardiac-related diagnostic procedures or surgeries. This includes education, cardiac monitoring, and personal care assistance.
